Comparative Study between Primary Surgical Intervention and Conservative Management in Cases of Jones Fracture of Base 5th Metatarsal Bone

Abstract
The fifth metatarsal base fracture is the most common fracture of the forefoot. The aim of this study was to evaluate the short term results of comparison between surgical and conservative management of Jones fracture of base 5th metatarsal: This is a comparative study included forty patients, twenty patients were treated by percutaneous fixation using cannulated 4.0 mm screw and twenty patients were treated conservatively with below knee cast. The duration from injury to admission to the hospital ranged from 1 to 3 days with an average of 2 days. The average follow-up duration was 6 months (range from 4 to 12 months). The American Orthopedic Foot and Ankle Society (AOFAS) used as a clinical rating system to monitor improvement following injury was done at 6 weeks, 12 weeks and 6 months. Patients treated with percutaneous screw fixation all achieved full union and had AOFAS score ranged from 91 to 98 at end of follow up at 12 weeks, while patients treated conservatively by cast had AOFAS score ranged from 73 to 87 at end of follow up at 12 weeks . After calculating P value to compare both groups according to end of follow up AOFAS Score P value = 0.000665781, P value significant < 0.001 which is highly significant. Early screw fixation is safe and effective method for treatment of acute Jones fracture of base of 5th metatarsal.
